A form can have many items.
Form items can be imported from a CSV file.
Usually do not apply Modifiers to codes. Just use the 5 digit HCPCS code separated by commas if multiple codes apply. See K0739RB exception below.
If Medicare labor code K0739, use code and modifier K0739RB.

- Part Number
- Description
- Codes: HCPCS codes (usually 5 digit no modifiers) separated by commas if multiple codes apply. If Medicare labor code K0739, use code and modifier K0739RB.
- Sub Code: Specifies category of K0108. Only use when Code is specified as K0108.
- Retail: Retail (MSRP).
- Detail Qty: Detail quantity must be a number such as 1 for each or 2 to specify a pair.
