Use if all items in the file pertain to a single modifier.

Actions
- Click Code drop down and enter Excel column name containing the code values.
- Click the appropriate Modifier drop down and enter Excel column name containing the allowable values.
- If file contains NU fees, use the NU Allow drop down.
- If file contains RR fees, use the RR Allow drop down.
- Click the green check button at the top of the dialog window.
